**How you’ll contribute **

- Coordinate patient care to ensure effective resource utilization, quality service delivery, and compliance with regulatory requirements.
- Conduct comprehensive assessments, develop and implement care plans, and evaluate patient progress to support individualized needs and preferences.
- Facilitate care coordination and communication across the interdisciplinary team, patients, and families.
- Lead discharge planning efforts to promote smooth transitions, continuity of care, and cost-effective outcomes.
- Advocate for patients by addressing their clinical, emotional, and social needs.
- Support utilization review and case management functions to enhance patient outcomes and satisfaction.
- Promote and uphold the hospital’s mission, vision, and values.

**Qualifications and requirements:**

- Current licensure as a **Registered Nurse, Social Worker, or other licensed healthcare professional** (e.g., Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist).
- **Certification in Case Management or Rehabilitation Nursing preferred** (e.g., CCM, ACM, ARN, RN-BC).
- **Minimum of 2 years of experience** in social work or case management in an inpatient setting preferred; acute or rehabilitation hospital experience strongly preferred.
- Strong **written and verbal communication skills** in English; additional languages are a plus.
- Proficiency with **basic computer applications** (e.g., Excel, Word, Outlook, PowerPoint).
- Excellent **organizational, time management, and analytical skills**, with the ability to work independently.
- Collaborative mindset with a strong **team-oriented approach**.
- Commitment to maintaining **confidentiality and professional standards**.
