- Exceptional communication, organization and customer service skills.
- The Nurse Navigator is responsible for enhancing the communication amongst the patient/family, the interdisciplinary team and all interdepartmental needs.
- Assists in the development of strategies to reduce LOS and re-admissions.
- Part of the cardiovascular team which monitors LOS and re-admission data for CHFAMI, COPD, and pneumonia.
- Monitors care process and develops approach to improve care efficiency and patient outcomes.
- Performs other related duties as assigned.

Schedule:

- Full time, Days
- 8am - 4pm

## Qualifications

**Required Education:**

- Bachelor's/Master's in Nursing or related field

**License/Certification**

- Current NJ (New Jersey) Registered Nurse License required
- BLS required

**Relevant Experience**

- 3 years of cardiology nursing
- Quality background preferred
