### What you'll do

- Conduct member health assessments
- Perform annual and change-in-condition reassessments
- Submit clinical documentation within 24 hours
- Coordinate care with interdisciplinary teams
- Travel to perform daily home visits

### Requirements

- Current unrestricted New York RN license
- Reliable transportation for travel
- Ability to travel up to 75 percent
- Case management experience
- Associate degree in nursing
- UAS NY experience or certification (preferred)
- Prior assessment nursing experience (preferred)
- Experience with Medicaid or LTSS populations (preferred)
