- Reside in Union County NJ
- Three years clinical practice experience
- Reliable transportation for field travel
- Ability to travel 75 percent of time
- Associate degree or diploma in Nursing
- LTSS experience (preferred)
- Case management or discharge planning experience (preferred)
- Managed Care experience (preferred)
