The Medicare Outpatient Observation Notice, the MOON, explains that an observation stay is outpatient, which matters because those days don't count toward the 3 inpatient days Medicare wants before it pays for skilled nursing. A third notice joined them in 2025, for patients moved from inpatient to observation.

- What's the difference between case management and utilization review?
Two jobs that share a department and get confused constantly. Case management coordinates the patient's care and discharge. Utilization review reads the chart against criteria to establish whether the stay is justified and will be paid for. Some hospitals combine them into one role, and a posting that does should say so, because the second is desk work of a very different kind.
- What does a hospital case manager do on discharge day?
Assembles everything that has to exist before a patient can leave: the bed at the next facility, the home health referral, the equipment delivered, the medication approved, the transport booked, and the family told. Then does it again when one of those falls through at 4pm. The skill is knowing who to call and how insistent to be.

The certified case manager credential is the one postings name, and it takes qualifying employment before you can sit it, so it comes after the first job rather than before. Most employers require a nursing or social work license underneath it to get in.
