PTCB wants a competency attestation signed by a qualified supervisor who has directly observed your training, skill assessment and competency assessment, first to award the certification and then every year. PTCB asks 2 more things of the signer: good standing with their employer and with every regulator over their site, and 5 years of working directly with compounded sterile preparations or supervising that work. Below 5 years the form still goes in, with a letter from the pharmacy director alongside it. Either way they have to have watched you work, so moving out of sterile compounding leaves nobody placed to sign.
