States split on what an aide may do with medication

States draw the line between reminding and administering differently. Florida counts opening the container, taking out the dose and placing it in the patient's hand as assistance with self-administration. Washington splits it by setting: a home health agency nurse may delegate applying, instilling or inserting a medication to a nursing assistant, while in-home delegation to a certified home care aide goes further and covers an insulin injection. New Jersey lets a nurse delegate a named medication to a certified aide, with the dose, route and review date written down.

How do home health aides clock in and out?

Electronically, at the client's home, on a phone app or a device in the house. The 21st Century Cures Act made states require electronic visit verification for Medicaid personal care from 2020 and home health from 2023, so the system records who arrived, where, and when. Ask an agency what happens when the app fails on a visit.

Can a home health aide change the plan of care?

No. It's the written list of what you do for that client, set by a nurse, and tasks outside it aren't yours to take on however sensible they look. If a family asks for something extra, that's a call to the office. The plan is also what protects you if anyone questions what happened on a visit.

Who supervises a home health aide?

A registered nurse, and on a skilled case another skilled professional on the plan, such as the therapist, may do it instead. Two checks get mixed up constantly: the supervisory assessment of the patient and the care, which federal rules let happen without the aide there, and the visit where somebody watches you work with a patient.
