Crisis shifts are a different kind of visit
Most hospice care is routine home care, paid by the day and delivered in visits of an hour or two. During a crisis Medicare pays for continuous home care instead: a minimum of 8 hours in the day, predominantly nursing, with aide and homemaker services alongside it. An aide on one of those is in the house for a shift rather than a visit, and the hours are billed hourly rather than folded into the daily rate. Agencies staff them from the team they already have.

- What's the hospice interdisciplinary team, and where does the aide fit?
The group that runs the plan of care together: nurse, aide, social worker, chaplain, physician and often a volunteer coordinator. Federal rules make them review each patient's plan at least every 15 days, and what the hospice aide reports goes into that review and changes what the team does next. The aide sits in the meeting as a member of the group.
- What happens if a patient dies while you're there?
You call the hospice nurse, not 911. Hospice patients have orders in place precisely so that an expected death isn't treated as an emergency, and the aide's job is to stay, keep the family company, and let the nurse take over when they arrive. Agencies train this specifically, and a good one debriefs you afterward.
- What does comfort care actually involve?
Bathing, mouth care, repositioning someone who can't move themselves, keeping skin intact and lips from cracking. It's the ordinary work of an aide, done for a different reason, and aides say the shift from fixing to comforting takes a while to settle into.
