Iowa's licensing department says the certification itself doesn't expire, and puts the 24-month clock on registry standing. The rule behind both is written by the health and human services department inside Iowa's Medicaid chapter, which then designates the Department of Inspections, Appeals, and Licensing to approve the training programs and to establish and maintain the registry. Reaching that registry is the aide's own errand either way. Anyone employed as a nurse aide in a Medicare or Medicaid certified nursing facility completes the registry application within their first 30 days of employment, and an aide who isn't employed can apply for inclusion by submitting a completed application.

The wait is counted differently in different buildings. In a nursing facility the aide needs to have finished an approved nurse aide course, a nurse aide training and testing program or a nurse aide competency examination, to be employed in that same facility and to have worked at least 480 hours there before the medication aide course starts, and to bring a letter of recommendation from the facility. An intermediate care facility for persons with mental illness runs the same 3 conditions with a different middle one, 6 consecutive months of employment in the same facility, and the rule adds that this requirement isn't subject to waiver.

There are 2, and both end at a challenge examination. Written documentation of certification as a medication aide in another state gets an applicant to a department approved nurse aide competency examination and the medication aide challenge examination, and the rule says the paragraph holding the hours at the same facility and the letter of recommendation doesn't apply to that person. A nursing student reaches the same challenge examination on 4 conditions of their own: a clinical or nursing theory course inside the previous 6 months, a nursing program pharmacology course inside the previous year, a written statement of competence from the pharmacology or clinical instructor, and the nurse aide competency evaluation. Either way the destination is the credential behind an Iowa medication aide job.
