Approval comes before any medical act
The Board of Nursing's rules in 21 NCAC 36 put the paperwork first. Among the things they require prior to the performance of any medical acts is a collaborative practice agreement with a primary supervising physician, and a nurse practitioner shall not practice until notification of approval to practice is received from the Board of Nursing after both Boards have approved the application. The agreement itself stays at the practice site; it is not submitted.

It ends when the agreement does.
