• Evaluate member needs and eligibility
• Coordinate and implement care plan activities
• Monitor care plan progress
• Provide member support and education
• Identify and escalate quality issues

Requirements

• Bachelor's degree in human-services field
• Two years of case management experience
• Microsoft Office and EHR experience
• Valid Illinois driver's license
• Managed Care experience (preferred)
• Experience with HIV and AIDS population (preferred)
