CVS Health is hiring for "Case Management Coordinator OhioRISE NE/NW/SE Regions" in Ohio. The job is full-time and hybrid at a clinic, 40 hours a week. Pay runs from $21.10 to $40.90 per hour. The schedule is Monday-Friday 8-5pm with flexibility needed.

What you'll do

• Evaluate member needs and eligibility
• Coordinate and implement care plans
• Collaborate with child and family teams
• Provide coaching and support to members
• Monitor care plan progress

Requirements

• Two years experience in behavioral health
• Two years experience in children's mental health
• Two years experience with Ohio delivery systems
• Ability to travel up to 50 percent
• Managed Care experience (preferred)
• Medicaid experience (preferred)
