What you'll do

• Evaluate member needs and eligibility
• Coordinate and implement care plans
• Monitor care plan progress
• Present cases at multidisciplinary conferences
• Provide coaching and support to members

Requirements

• Two years case management experience
• Experience with serious mental illness
• Experience working with elderly
• Reside in West Valley Arizona
• Travel up to 50 percent
• Experience with physical disabilities (preferred)
• Experience collaborating with medical professionals (preferred)
