Primary Duties and Responsibilities

Position Titles included:

  • Clinical Nurse Navigator I
  • Community Nurse Navigator I
  • MBS Nurse Navigator I
  • Ortho Navigator RN

The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration sensitive diagnoses. Serves as a patient advocate who coordinates care in conjunction with the physicians from time of diagnosis or admission through discharge or up to end of life. Educates patient and family/significant others on the health care experience; implements, coordinates, monitors and evaluates care options, services and resources. Links family/patient to appropriate resources as needed. Proactive in finding ways to enhance the patient experience clinically and in service. Provides an exceptional patient experience. Assists in quality improvement initiatives by gathering, tracking, and researching quality metrics. Monitors outcomes, e.g. patient satisfaction, compliance with treatment regimen and loyalty. Interacts with department Nurses, may orient and train incoming Clinical Nurses and Patient Care Assistants. Performs other duties as assigned.

Oncology – In addition to the above: the Navigator may be responsible for working in conjunction with the Research team to identify and navigate patients into clinical trials.

Metabolic and Bariatric Surgery (MBS) – In addition to the above: Under MBS Director leadership, is primary liaison between hospital and MBS proceduralists, adhering to MBS and Quality Improvement Program (MBSAQIP) Standards, including attending minimum required meetings per year, encompassing MBS Annual Program Review, and assisting with: coordination of MBS Leadership Committee, program development and accreditation, development of staff/patient education, clinical pathways and protocols, monitoring patient outcomes and providing feedback for quality improvement initiatives.

Community Health – In addition to the above: Works with low income, uninsured and underinsured patients. The Nurse Navigator will provide case management and coordination of services in the community by identifying medical, psycho-social and economic conditions to ensure that appropriate treatment and services are obtained. Develops health related educational programs and deliver lectures or perform demonstrations. Provides health and wellness coaching, if certified to do so.

Women’s COE – In addition to the above: Individualized support and guidance to expectant mothers and their families throughout the perinatal journey. Coordinates care, educates patients on pregnancy and childbirth, and assists with navigating/communicating with all team members involved in the patients’ journey. Serves as a trusted resource, advocate, and liaison between patients and healthcare providers, ensuring that each patient receives comprehensive and personalized care. Plans and runs multidisciplinary meetings and puts together care conferences.

Brain Health Outreach Program (BHOP) – In addition to the above: Under Center for Brain Health leadership, performs cognitive screenings, facilitates access to dementia services, develops dementia education for both community and physician audiences, acts as internal and community-facing dementia liaison and collaborator, and assists in development of CBH volunteer program. 

Qualifications

Education and Experience

Minimum of 5 years’ experience in nursing and 3 years’ of experience in area of specialty required.

Additional department specific requirements are as follows:

  • Acute Rehab: Prior Navigator experience preferred.
  • Care Management: Prior Sepsis RN experience preferred. Prior experience as a Case Manager, Navigator, or Health Educator preferred.
  • Community Health: BSN required. Bilingual skills, especially Spanish are preferred.
  • GI Lab: BSN required
  • Hoag Orthopedic Institute (HOI): Prior Orthopedic Navigator Experience preferred.
  • Hoag Medical Group: BSN required.
  • Oncology: One plus (1+) years of Oncology nursing experience required; Prior Navigator experience preferred. Prior research experience is preferred.
  • Neurosciences: Fulfills mandatory stroke education requirements per certification agency
  • Brain Health Outreach Program (BHOP)/Memory: 5+ years’ of experience in Dementia, Geriatrics, Public Health, or Case Management. 
  • Women’s COE: BSN required

License Required

Registered Nurse in the State of California - Current and in good standing.

License Preferred

Certifications Required

  • Acute Rehab: Certified Registered Rehabilitation Nurse (CRRN) required.
  • Community Health: BLS issued by the American Heart Association
  • Gamma Knife: ACLS and PALS certification is required within 30 days of hire.
  • Neurosciences Administration/Pain Management: BLS Required
  • Women’s COE: BLS issued by the American Heart Association
  • Breast Care Centers: BLS issued by the American Heart Association 
  • Heart Valve Clinic:  BLS issued by the American Heart Association

Certifications Preferred

  • Current BLS certification
  • Community Health: Health Coaching Certification
  • Hoag Orthopedic Institute (HOI): Active NAON member and certified ONCB or Medical-Surgical certification; ACLS Certification.
  • Neurosciences (Stroke): AANN (American Association of Neuroscience Nurses): SCRN or CNRN or ANVC (Association of Neurovascular Clinicians): NVRN-BC or ASC-BC
  • Metabolic and Bariatric Surgery: Certified Bariatric Nurse (CBN)
  • Oncology Administration: Oncology Nursing Certification (OCN) preferred
