Community Health Worker. On-site, New York

CVS Health is hiring a Community Health Worker in New York. The job is full-time at a clinic, 40 hours a week. Pay runs from $18.50 to $42.35 per hour.

What you'll do

• Connect patients to community resources
• Facilitate communication between care parties

• One year experience in social services
• Access to reliable transportation
• Proficient computer skills
• Strong oral and written communication
