What you'll do

• Serve as primary member point of contact
• Build relationships through proactive outreach
• Help members navigate healthcare services and benefits
• Coordinate referrals and member routing

Requirements

• 3+ years in patient advocacy or care coordination
• 2+ years working with oncology patients
• High School Diploma or equivalent
• Oncology patient navigation experience (preferred)
• Experience with complex or chronic conditions (preferred)
• Bilingual language skills (preferred)
