What you'll do

• Attend case rounds to coordinate member care
• Educate care teams on recovery support
• Collaborate with healthcare and justice professionals
• Assist members with health and social goals

Requirements

• Experience with justice-involved individuals
• One to two years community or outpatient experience
• Proficiency in MS Office Suite
• Ability to work independently
• Strong communication skills
• Bachelor's degree in Human Services (preferred)
• Managed Care setting experience (preferred)
• Bilingual skills (preferred)
