What you'll do

• Conduct phone and video patient consultations
• Identify and resolve medication related problems
• Assess medication indication effectiveness and safety
• Create medication summaries and care plans
• Monitor patient drug therapy outcomes

Requirements

• Reside in Arizona
• Proficient in Microsoft Office
• Work in deadline-oriented environment
• Strong organizational and prioritization skills
• One to two years clinical experience (preferred)
• Clinical and analytical problem solving exposure (preferred)
