What you'll do

• Assess and diagnose patients at clinics
• Oversee and direct primary care

Requirements

• Two years post-training primary care experience
• Experience serving Medicare or senior population
• Commitment to role for two years
• Minimum 24 DPC hours per month
• Prior Oak Street Health employment
• Familiarity with value-based care metrics (preferred)
