Registered Nurse. Remote, Texas

CVS Health is hiring a Registered Nurse in Texas. The job is full-time and remote, 40 hours a week. Pay runs from $54k to $143k per year. The schedule is Monday to Friday, 7am to 6pm, no weekends.

What you'll do

• Conduct telephonic health assessments
• Develop individualized care plans
• Coordinate member care and referrals
• Manage complex and co-morbid conditions
• Collaborate with multidisciplinary teams

Requirements

• Five years clinical RN experience
• Associate Degree in Nursing
• Work Monday through Friday business hours
• Six months case management experience (preferred)
• Case Management Certification (preferred)
• Bachelor of Science in Nursing (preferred)
