What you'll do

• Coordinate transitions between care settings
• Facilitate interdisciplinary care team meetings
• Conduct post-discharge member assessments
• Provide medication management and education

Requirements

• Three years of clinical practice experience
• One year of care coordination experience
• Proficiency in Microsoft Office Suite
• Access to private dedicated workspace
• Certified Case Manager (preferred)
• Experience in home health care coordination (preferred)
• Bilingual (preferred)
