What you'll do

• Assess and plan member care needs
• Coordinate case management for chronic conditions
• Act as liaison between members and providers
• Document all case work activities
• Provide educational and prevention information

Requirements

• Reside in Florida
• 3+ years clinical practical experience
• 2+ years case management experience
• Associate's Degree in Nursing
• Hardwired internet connection
• Bachelor's Degree (preferred)
• Certified Case Manager (preferred)
• Experience with Medicare members (preferred)
