What you'll do

• Assess post-discharge member needs
• Facilitate transitions between care settings
• Coordinate interdisciplinary care team meetings
• Educate members on medications and resources

Requirements

• Three years clinical practice experience
• One year care coordination experience
• Proficiency in Microsoft Office Suite
• Access to private dedicated workspace
• Certified Case Manager (preferred)
• Three years care management experience (preferred)
• Bilingual (preferred)
