Registered Nurse. Remote, Texas

CVS Health is hiring a Registered Nurse in Texas. The job is full-time and remote, 40 hours a week. Pay runs from $61k to $130k per year. The job is in Aetna National Medical Excellence. The schedule is Monday-Friday, 8:00 AM-5:00 PM.

What you'll do

• Assess and manage complex medical care
• Develop proactive patient care plans
• Identify clinical and social care barriers
• Collaborate with multidisciplinary healthcare teams
• Conduct telephonic care management and coaching

Requirements

• 6+ months of case management experience
• Active unrestricted Compact RN license
• Associate Degree in Nursing
• CCM certification (preferred)
• Transplant nursing experience (preferred)
• BSN degree (preferred)
