Registered Nurse. Hybrid, Louisiana

CVS Health is hiring a Registered Nurse in Louisiana. The job is full-time and hybrid in patients' homes, 40 hours a week. Pay runs from $54k to $117k per year. The schedule is Variable work scheduled with ability to work 2 days weekly until 9pm local time.

What you'll do

• Coordinate case management activities for members
• Evaluate member medical needs
• Develop wellness strategies for members
• Instruct programs per clinical policies
• Document activities per regulatory standards

Requirements

• Reside in specific Louisiana parishes
• Case management experience
• Strong customer service and problem solving
• Excellent telephonic communication skills
• Ability to travel 125 miles round trip
• Bachelors or Masters degree (preferred)
• Managed care organization experience (preferred)
