State protocols decide this, and they have widened. Naloxone reaches patients everywhere without a prescriber, though that's over-the-counter access now and not a pharmacist authority. Guttmacher counted 30 states and DC allowing pharmacists to prescribe hormonal contraception in April 2026. Test-and-treat authority is narrower, reaching more than 20 states by early 2026: there a pharmacist can test for something like influenza or strep and prescribe on the result without a collaborating prescriber. Idaho's rule is among the broadest, written by category rather than one condition at a time.

Where do pharmacists work besides a pharmacy counter?: Insurers and pharmacy benefit managers hire them for coverage review. Drug information services answer clinical questions from prescribers, and specialty and mail-order pharmacies hire for verification. In ambulatory clinics a pharmacist runs medication management under a collaborative practice agreement with a physician. Most of that work keeps weekday hours.
Do I need a residency to work as a pharmacist?: Not for community or retail practice, where the license is all you need. Clinical hospital roles are the other case: a postgraduate year one residency, written PGY1 in postings, is the usual route in, and specialties add a second year on top of it. Do pharmacists need a license in every state they work in?: Yes, one per state, though you rarely retake the whole exam. Most states run a license transfer through the NABP, the association of state boards, and add a law exam of their own. California is the strict case, since it makes a transferring pharmacist sit its own exam and won't count a NAPLEX passed before 2004.
