Case Manager. On-site, Ohio

CVS Health is hiring a Case Manager in Ohio. The job is full-time at a hospital, 40 hours a week. Pay runs from $61k to $130k per year.

What you'll do

• Assess member needs and eligibility
• Develop and monitor care plans
• Perform crisis intervention
• Coordinate care with providers
• Provide member coaching and support

Requirements

• 3-5 years direct clinical practice experience
• Master's degree in Behavioral or Mental Health
• Crisis intervention skills (preferred)
• Managed care or utilization review experience (preferred)
