Case Manager. Remote, Louisiana

CVS Health is hiring a Case Manager in Louisiana. The job is full-time and remote, 40 hours a week. Pay runs from $54k to $117k per year.

What you'll do

• Conduct outreach following inpatient and emergency visits
• Facilitate timely follow-up appointments
• Improve member engagement with treatment plans
• Address barriers to accessing care
• Connect members with community resources

Requirements

• Prior clinical or managed care experience
• Bachelors degree
• Behavioral Health experience (preferred)
• Managed Care experience (preferred)
