What you'll do

• Manage patient transitions across care settings

Requirements

• Two years transitional social work experience
• Knowledge of Medicare and Medicaid
• Access to reliable transportation
• Proficiency in Microsoft Office Suite
• Experience in utilization management (preferred)
• Spanish language proficiency (preferred)
