Discharge planning is a named hospital job
Medicare's conditions of participation put the discharge planning evaluation and the plan itself in the hands of a registered nurse, a social worker or other appropriately qualified personnel. The same rule says the hospital has to help the patient choose what comes next, using quality and resource use data on home health agencies, skilled nursing facilities, inpatient rehabilitation and long-term care hospitals, hand the list of participating providers over and document that it did, and never narrow the choice for them. A hospital posting saying discharge planning is describing that.

2Your questions about clinical social worker jobs, answered
- What do LMSW and LCSW mean in a job posting?
There are 2 license levels. Licensed master social worker comes with the degree and lets you practice under supervision; licensed clinical social worker comes after a period of supervised post-graduate hours and allows independent diagnosis, therapy and insurance billing. Postings that say LCSW required mean it. Postings that say LMSW should say who provides the supervision and whether the employer pays for it.
- What's medical social work?
Medical social work is the hospital version of the job: psychosocial assessment, family meetings, guardianship, abuse reporting and the discharge work, all at the pace of the unit. Hospitals, hospice and dialysis all employ clinical social workers on it.
- Do clinical social workers only do therapy?
Public agency work is the other half of the profession. Child welfare, adult protective services, schools and veterans' services hire the same license for assessment, court reporting and coordination, sometimes with no therapy in the job at all, and they carry heavier caseloads for it.
