Community Health Worker. On-site, Indiana

CVS Health is hiring a Community Health Worker in Indiana. The job is full-time at a clinic, 40 hours a week. Pay runs from $18.50 to $31.72 per hour.

What you'll do

• Connect patients to community resources
• Facilitate communication between patients and providers
• Document patient interactions in medical records
• Provide culturally appropriate health education
• Manage patient care plans with social workers

Requirements

• One year experience in social services
• Proficient computer skills
• Access to reliable transportation
• Ability to travel within community
• US work authorization
• Bilingual English and Spanish (preferred)
• Community Health Worker certification (preferred)
• Experience with electronic medical records (preferred)
