Community Health Worker. Remote, Louisiana

CVS Health is hiring a Community Health Worker in Louisiana. The job is full-time and remote, 40 hours a week. Pay runs from $18.50 to $31.72 per hour. The schedule is Monday-Friday, 8am-5pm, with flexibility.

What you'll do

• Conduct home visits for chronic condition support
• Educate providers on social determinants of health
• Build relationships with members and care teams
• Assist with care plan implementation and education
• Maintain electronic health records and documentation

Requirements

• Reside in specific Louisiana regions
• Access to reliable transportation
• One to two years clinical support experience
• One year community outreach experience
• One year experience with clinical needs clients
• Community or outpatient setting experience (preferred)
• Mental Health First Aid training (preferred)
• Experience serving Medicaid populations (preferred)
