What you'll do

• Provide clinical medication counseling and reassessments
• Perform medication therapy management
• Coordinate medication adjustments
• Supervise specialty pharmacy care coordinators
• Facilitate patient access to complex therapies

Requirements

• Bachelor's degree or Doctorate of Pharmacy
• Kentucky pharmacy license
• 1-3 years related work experience (preferred)
• ASHP accredited pharmacy residency (preferred)
• Ambulatory clinical pharmacy practice experience (preferred)
