What you'll do

• Provide clinical medication counseling
• Manage chronic disease states
• Perform medication therapy management
• Supervise specialty pharmacy care coordinators
• Facilitate patient access to specialty medications

Requirements

• Bachelor's Degree in Pharmacy or PharmD
• Kentucky pharmacy license
• 1-3 years related work experience (preferred)
• ASHP accredited pharmacy residency (preferred)
• Ambulatory clinical pharmacy experience (preferred)
