What you'll do

• Assess member health status and care needs
• Develop and implement case management plans
• Conduct comprehensive clinical assessments
• Advocate for patients regarding health coverage
• Provide coaching and education to members

Requirements

• 3+ years of Microsoft Suite experience
• Direct hardwired internet connection
• Minimum 25 Mbps download and 3 Mbps upload
• Private designated home work location
• Compact RN License (preferred)
• Managed Care experience (preferred)
• BSN degree (preferred)
