Registered Nurse. Hybrid, Illinois

CVS Health is hiring a Registered Nurse in Illinois. The job is full-time and hybrid in patients' homes, 40 hours a week. Pay runs from $67k to $143k per year.

What you'll do

• Assess and coordinate member healthcare services
• Develop individualized care plans for members
• Facilitate access to community and health resources
• Conduct in-person visits in various settings
• Collaborate with interdisciplinary care teams

Requirements

• Three to five years clinical nursing experience
• Two to three years case management experience
• Reside in Logan County Illinois
• Ability to travel within assigned geographic area
• Proficiency in Microsoft Office applications
• Certified Case Manager certification (preferred)
• Experience with Medicare or Medicaid populations (preferred)
• Bachelor degree in nursing (preferred)
