Registered Nurse. Hybrid, New Jersey

CVS Health is hiring a Registered Nurse in New Jersey. The job is full-time and hybrid in patients' homes, 40 hours a week. Pay runs from $67k to $143k per year. The schedule is Monday, Friday, 8:00 AM, 5:00 PM.

What you'll do

• Conduct in-home assessments for MLTSS members
• Evaluate medical behavioral and social needs
• Collaborate with providers and interdisciplinary teams
• Document assessments in electronic health records

Requirements

• Three years of clinical experience
• Ability to travel 75% of the time
• Associate’s degree in Nursing
• Reliable transportation
• Case management or care coordination experience (preferred)
• Managed care environment experience (preferred)
• BSN degree (preferred)
