Registered Nurse. Remote, Pennsylvania

CVS Health is hiring a Registered Nurse in Pennsylvania. The job is full-time and remote, 40 hours a week. Pay runs from $61k to $130k per year. The job is in Case Management team. The schedule is Monday, Friday, 8:00 AM, 4:30 PM.

What you'll do

• Conduct telephonic member health assessments
• Develop individualized care plans
• Coordinate care across providers and resources
• Review clinical information and claims history
• Collaborate with multidisciplinary teams

Requirements

• 1+ year of Microsoft Office experience
• Ability to work rotating late shifts
• Ability to participate in holiday on-call
• Associate Degree in Nursing
• Compact RN license (preferred)
• Case Management experience (preferred)
• BSN (preferred)
