Registered Nurse. Remote, New Jersey

CVS Health is hiring a Registered Nurse in New Jersey. The job is full-time and remote, 40 hours a week. Pay runs from $67k to $143k per year. The job is in Community Care team. The schedule is Monday - Friday 8:00 AM - 5:00 PM Eastern Time.

What you'll do

• Coordinate care for chronically ill members
• Assess medical and vocational status
• Develop and monitor care plans
• Liaise with providers and families
• Document all case activities

Requirements

• 3+ years clinical practice experience
• 2+ years case management experience
• Proficiency with Microsoft Office suite
• Ability to travel up to 10%
• Associate's Degree in Nursing
• Experience with Medicare members (preferred)
• Managed care experience (preferred)
• Bachelor's Degree in Nursing (preferred)
