Registered Nurse (RN) Care Manager – Lakewood Family Medicine 

Job Type: Full-Time | 0.8 FTE or 64 hours/pp | Exempt/Salaried

Schedule: 8 x 8-hour Day shifts/pp, Flexible Start Times

As an RN Care Manager, you'll build meaningful patient relationships, coordinate care across the healthcare continuum, and help patients achieve better health outcomes. How you'll contribute

A Care Manager RN who excels in this role:

  • Identifies the targeted population and implements clinical interventions and protocols.

  • Collaborates with PCP, patients and members of health care team including continuum of care settings and community resources.

  • Provides self-management support and includes patient/family as active members in the team approach.

  • Facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.

  • Coordinates care between PCP and acute care hospitals, specialists and post -acute care services.

What we're looking for

Applicants should have a current Michigan RN license and possess an associate’s degree from an accredited nursing school (bachelor’s preferred). 

Additional requirements include

  • Basic Life Support Health Care Provider (BLS-HCP)

  • 2 years in adult medicine and pediatric patients
