Schedule: 8 x 8-hour Day shifts/pp, Flexible Start Times

Make a difference beyond the exam room at Lakewood Family Medicine in Harvey, Michigan. As an RN Care Manager, you'll build meaningful patient relationships, coordinate care across the healthcare continuum, and help patients achieve better health outcomes. How you'll contribute

A Care Manager RN who excels in this role:

• Identifies the targeted population and implements clinical interventions and protocols.

• Collaborates with PCP, patients and members of health care team including continuum of care settings and community resources.

• Provides self-management support and includes patient/family as active members in the team approach.

• Facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.

• Coordinates care between PCP and acute care hospitals, specialists and post -acute care services.

What we're looking for

Applicants should have a current Michigan RN license and possess an associate’s degree from an accredited nursing school (bachelor’s preferred). 

Additional requirements include

• Basic Life Support Health Care Provider (BLS-HCP)

• 2 years in adult medicine and pediatric patients
