Who pays an independent assistant
Medicare recognizes a physician assistant, nurse practitioner or clinical nurse specialist as a billable assistant at surgery, and doesn't recognize a certified surgical assistant. Independent work therefore runs on commercial plans, and each one sets its own rule about paying a non-physician. The fee schedule also flags which procedures allow an assistant at all. That's the difference between an independent contract and a hospital paycheck.

- Who employs a surgical assistant?
There are 2 models with a grey area between them. Hospitals employ most surgical assistants onto the operating room staff like any other member of it. Others work independently and bill as an assistant at surgery, which makes the payer mix and the contract terms matter as much as the clinical work. Surgical assistant groups and surgeons' own practices sit somewhere in between.
- What does first assisting add beyond passing instruments?
You have your hands in the field. Retraction, exposure, controlling bleeding, suturing and helping close are the assistant's work, so the surgeon can operate while somebody else manages the wound. A surgical technologist keeps the field and the instruments; the assistant works in the wound alongside the surgeon.
- Can a surgical technologist move into first assisting?
It's the usual route, and the gate is the credential. Only graduates of an accredited first assistant program can sit the CSFA exam, and those programs take you in on a CST plus at least a year in the operating room, with a documented case log where the program asks for one.
