Clinical Social Worker. Remote, Kentucky

Baptist Health is hiring a Clinical Social Worker in Kentucky. The job is full-time and remote. The schedule is Monday through Friday 8am to 4:30pm.

What you'll do

• Conduct telephonic patient outreach
• Develop personalized care plans
• Coordinate community resources
• Provide emotional support services
• Facilitate advanced care planning

Requirements

• Master’s degree in social work
• Kentucky Certified Social Worker license
• Residency in KY or IN
• Two years medical social work experience (preferred)
