Community Health Worker. On-site, New York

What you'll do

• Connect patients to community resources
• Facilitate communication between care team and patients
• Document patient interactions in medical records
• Provide culturally appropriate health education
• Manage patient referrals and care plans

Requirements

• One year experience in healthcare or social services
• Proficient computer skills
• Access to reliable transportation
• US work authorization
• Community Health Worker certification (preferred)
• Associates or Bachelors degree (preferred)
• Experience with electronic medical record systems (preferred)
