Community Health Worker. On-site, Louisiana

CVS Health is hiring a Community Health Worker in Louisiana. The job is full-time in patients' homes, 40 hours a week. Pay runs from $18.50 to $31.72 per hour. The schedule is Monday to Friday, 8am to 5pm, with two days weekly until 9pm.

What you'll do

• Conduct home visits with members
• Educate healthcare professionals on social determinants
• Assist with care plan implementation
• Help members navigate community services
• Teach disease self-management

Requirements

• Three to five years relevant work experience
• One to two years clinical support experience
• One year community outreach experience
• One year experience with clinical needs
• Access to reliable transportation
• Experience in community or outpatient setting (preferred)
• Mental Health First Aid training (preferred)
• Trauma Informed Care training (preferred)
