What Your Day Might Look Like

- Conduct comprehensive evaluations and develop patient-centered treatment plans in the home of each patient - Collaborate with physicians, case managers, and fellow rehab team members - Communicate with patients and families to ensure understanding and continuity of care - Support various appointment types and adapt care plans as needed

Qualifications

Essential Functions/Responsibilities:

• Evaluate and treat patients in their homes

Minimum Education:

• High School Diploma or GED, required
• Bachelor's in Physical Therapy, required
• Master or Doctorate in Physical Therapy preferred

Minimum Experience:

• 1 year of experience in homecare, strongly preferred

License/Certification:

• NJ License in Physical Therapy, required
• BLS, preferred
• Valid and unrestricted Driver License with a clean driving record, required.
