A patient can start physical therapy without a physician's referral in every state. As of mid-2025 there was no limit attached in 21 of them. The other 29 and the District of Columbia are provisional: a visit or time limit before a referral is required, or a referral needed for a specific test or intervention such as needle electromyography or spinal manipulation. Insurance decides separately: a health plan can require a referral for payment where the practice act doesn't. Together they decide how patients arrive at an outpatient clinic.

Productivity is a target for how much of your paid day is billable treatment, quoted as visits per day or as a percentage. That number decides whether documentation happens between patients or after hours, and it varies more between employers than pay does.

How does outpatient physical therapy differ from acute care?

Outpatient sees the same patients over weeks and works toward a goal you both agreed. Acute care in a hospital sees them once or twice, often the day after surgery, and the question is whether they're safe to move and safe to go home. Skilled nursing sits between the two with longer stays and a heavier documentation load.

Do new graduate physical therapists get mentorship?

Some employers build it in and say so: scheduled time with a senior therapist, a lighter early caseload, and support toward board specialization. Others hand you a full schedule in week one.
