What you'll do

• Assess member health status and needs
• Develop and implement case management plans
• Advocate for patients within coverage limits
• Monitor and evaluate care plan outcomes
• Provide coaching and education to members

Requirements

• 3+ years of Microsoft Suite experience
• Ability to obtain multi-state licensure
• Compact RN License (preferred)
• Case Management experience (preferred)
• Managed Care experience (preferred)
