Registered Nurse. On-site, New York, NY

CVS Health is hiring a Registered Nurse in New York, NY. The job is full-time in patients' homes, 40 hours a week. Pay runs from $67k to $143k per year. The schedule is Monday to Friday, 8:30 a.m. to 5:00 p.m.

What you'll do

• Conduct member health assessments
• Perform annual and change-in-condition reassessments
• Submit clinical documentation within 24 hours
• Coordinate care with interdisciplinary teams
• Travel to perform daily home visits

Requirements

• Current unrestricted New York RN license
• Reliable transportation for travel
• Ability to travel up to 75 percent
• Case management experience
• Associate degree in nursing
• UAS NY experience or certification (preferred)
• Prior assessment nursing experience (preferred)
• Experience with Medicaid or LTSS populations (preferred)
