Social Worker (MSW) Per Diem, Shift/Hours Varies, Atlantic Health Chilton Medical Center, NJ

Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center. 

Essential Function/Responsibilities:

• Provide Social Work coverage to facilitate discharges, meet with patients and families to assess and provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities.
• Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities. 
• Phone, Fax and Electronic documentation a must. 

Qualifications

Minimum Education:

• MSW and NJ LSW  

Minimum Experience:

• 1-year medical setting (acute care hospital) or 1-year externship would be preferred
