The J-1 waiver behind some sponsorship offers

About 25% of physicians in practice trained outside the United States and Canada, and nearly all of them did a US residency; since 2023 more than a dozen states have opened licensure routes that skip it. A physician who trained here on a J-1 owes 2 years back home, and the usual way out is a waiver granted against 3 years of service in a designated shortage area. Each state gets 30 places a year, on an authorization Congress has to keep renewing.

What's the difference between board-eligible and board-certified?

The specialty exam is the difference. For a physician, board-eligible means residency is finished and the exam isn't, which is a normal state for a new graduate and one most employers will hire into with a deadline attached. Board-certified is the finished article, and hospital privileges, insurance panels and partnership tracks tend to be written around it.

What's credentialing and why does it delay a start date?

The verification a hospital does before letting you treat patients: training, licenses, malpractice history, references, all checked at source, then a committee vote on privileges. Payer enrollment runs alongside it and can take longer still, so a signed contract is months short of a start date.

How does academic practice differ from community practice?

They differ in what you're measured on. An academic post attaches teaching, research and residents to the clinical work, usually at lower clinical pay and with protected time that varies from generous to theoretical. Community practice is clinical throughput, often with production-based compensation, and no obligation to publish. Academic pay assumes the teaching and the research are part of the week; whether they fit inside it's the question to ask.
